Travel Registration
The following are to be returned, no later then 6/27/09, to register your child for the Fall 2009 season:

RG-6 form; please print clearly, sign, and date.

Palumbo SC player info/medical release form; please print clearly, sign, and date.
-
a 1”x 1” recent headshot photo (for player pass).
-
proof of date of birth (new players only): a copy of a birth certificate or passport.
-
registration fee of $100 ($75 if not getting a new uniform), check or money order payable to Palumbo Soccer Club; for the U14 and older teams only, due to increased ref fees, registration fee of $125 ($100 without new uniform); additional siblings: 10% discount; requests for reduced registration fees should be directed to Vicki: maxsmom94@comcast.net.
