Travel Registration
You must respond by email no later than 6/29 if your child will be accepting a spot offered to him/her. The following forms and information must be returned by 7/6 to register your child for the Fall 2011 season (a $25 late fee will be charged for any forms received after 7/20). Here is the required information:

RG-6 form; please print clearly, sign, and date.

Palumbo SC player info/medical release form; please print clearly, sign, and date.

a 1”x 1” recent headshot photo (for player pass).

proof of date of birth (new players only): a copy of a birth certificate or passport.

registration fee of $120 ($85 if not getting a new uniform), check or money order payable to Palumbo Soccer Club; for the U14 and older teams only, due to increased ref fees, registration fee of $150 ($110 without new uniform); additional siblings: 10% discount; requests for reduced registration fees should be directed to Sari Love: roselovemg@aol.com.
